Authorization Form

Occasionally, you may not be able to appear in person to pick up your Goethe Card or a similar document. You can authorize another person to do this for you using this form.

Please note: This authorization also includes the right to receive documents and certificates. If authorization is to be granted for multiple items, please issue separate authorization forms in each case.
